A Time to Celebrate and A Time To Prepare

Your son’s transition from a structured treatment environment back into daily life is a milestone to celebrate, but it can also be a challenging experience for your son and you alike. Our team is here to support both you and your son every step of the way.

Transition planning starts three months prior to a recommended discharge. In this stage, our entire therapeutic team helps your son and your family prepare for his transition back into the community. Transition plans are highly individualized as is the transition planning process. Whatever your son’s looks like, we will help your entire family through this process to make this last stage of the process as seamless as possible.

Our goal is to help your child maintain the progress they’ve made while in treatment and ensure a smooth transition back into his community. To achieve this, we offer a range of support services and resources, including:

Aftercare planning: We’ll work with your child and your family to develop a personalized aftercare plan that meets their specific needs and helps them maintain their recovery journey.

Continuing care groups: Our continuing care groups provide a supportive environment for your child to connect with others who understand what they’re going through and provide encouragement and motivation as they continue their journey.

Connections to community resources: We’ll help your child connect with resources in the community, such as local support groups and therapy services, to ensure they have the support they need to maintain their progress.

Ongoing communication with your child’s treatment team: Your child’s treatment team will remain available to offer support and guidance as they make the transition back into daily life.

We are invested in the long-term success of your son and reintegrating back into the home, on to college, or into the workforce are all landmark events that require being prepared and ready for the challenegs each of those situations will bring.
